What we provide
Developmental disability supports
Mapped to plan service categories on our DDA Services page.
- Community residential support — supported living with assistance in daily activities, health management and personal care.
- Day enrichment — structured activities and individualised programmes building skills, confidence and community participation.
- Caregiver relief — planned respite so family caregivers can rest, with continuity of care maintained.
- Community empowerment and development — advocacy, skills development and outreach that support independence and inclusion.
- Day programmes and therapy sessions — in-house or through other licensed and regulated providers, against the person’s agreed objectives.
Nursing support
- In-community nursing support — nursing delivered where the person identifies as their personal community.
- Residential nursing support — nursing in the least restrictive residential setting that meets the person’s needs.
- Health assessments, medication administration and management, wound care, post-operative support, chronic disease monitoring, rehabilitation assistance and ongoing health supervision.
Private duty and geriatric care
- Individually arranged care for older adults — assistance with daily living, medication management, mobility support, health monitoring and companionship.

How to make a referral
Call or email us directly. We will confirm receipt, tell you plainly whether we can take the referral, and say what happens next. If we cannot help, we will tell you promptly rather than leave a family waiting.
What helps us respond quickly
- The person’s name, contact details, and the best person to reach first.
- Your name, role and contact details.
- The services being sought, and the intended start date.
- The current plan of service or assessment, where one exists.
- Relevant health information, including medications and known risks.
- The expected funding route.
You can also use our referral form. Please send documents containing personal health information by a secure route rather than ordinary email. Call us and we will agree the safest method with you.
How we work
- Informed choice. We understand that the choice of provider belongs to the person and their family. We will never present ourselves as the only option, and we support a person’s right to change provider.
- Person-centred planning. Services are built around the goals the person decides, their abilities and their own description of what they need — not around a fixed menu.
- Least restrictive setting. We support people in the setting that meets their needs with the least restriction on their life.
- Non-discrimination. We are a faith-based organisation, and we serve everyone who is eligible regardless of faith, background or belief. Services are never conditional on religious participation.
- Working with your team. We coordinate with coordinators, clinicians, families and other providers, and we keep you informed of material changes.
